In 2010, a group of organizations decided to work with Dr. Krause to look into why fatal injury rates across organizations and industries had stopped declining and were sometimes even increasing, while recordable injury rates had improved consistently over the decades. This was a disturbing trend. It meant that the things we were all doing to reduce workplace injuries were working for minor injuries but they were not working for Serious Injuries and Fatalities (SIFs). It meant that we needed to rethink how we were approaching safety.
